How to register:

You may register in one of three ways:
Online: available after 6th November - payment by credit card* only
Fax/Mail: complete the registration form and fax to +34.93.301.12.55 or mail it to AOPC (congress/@/aopc.es). Payment by check or credit card *
* Credit card payments: Visa and MasterCard are accepted only via fax.
Important notice: in order to avoid double booking please send your forms only once. 

IMPORTANT NOTICE: We remind authors with accepted abstracts that they should register prior to April 15th so that they can be included in the book of abstracts.

REGISTRATION ENTITLEMENTS

Full: Attendance at scientific sessions and exhibition, congress kit with final programme and abstract book, coffee breaks, luncheons, welcome reception, free city transportation pass and certificate of attendance.

Reduced: Attendance at scientific sessions and exhibition, congress kit with final programme and abstract book, coffee breaks, luncheons on Monday and Tuesday, welcome reception, free city transportation pass and certificate of attendance.

Please note: the reduced fee is reserved for students and delegates from low income countries. Because verification is required, the following papers must be submitted together with the registration form (mail or fax):

  • Students: a letter written on official letterhead and signed by a department head or mentor
  • Delegates from developing countries: a copy of the passport

One-Day: Attendance at scientific sessions and exhibition on that day, congress kit with final programme and abstract book, coffee breaks and luncheon on that day, and certificate of attendance (welcome reception not included)

MEMBERSHIP RENEWAL

Please note: Members must renew their memberships by 31st July 2007 if they wish to continue receiving membership benefits (e.g., access to the International Journal of Technology Assessment in Health Care). Those choosing to renew their memberships after 31 July 2007 will need to register at the full new member rate.

PAYMENT, CANCELLATION AND DATA PRIVACY POLICY

Payment and Confirmation

Registration fees are in Euros, and Include VAT.

Payment must accompany your registration form, either by check, credit card or bank draft and be in Euros. Registrations without payment will not be processed. Upon receiving a complete registration form, AOPC will send out a confirmation of receipt within three (3) working days. The final confirmation of registration will be sent only after confirmation of payment from the bank/credit card company has been received.

Cancellation Policy

All cancellations must be in writing and faxed or e-mailed to AOPC.

Before 15th April 2007 full refund excluding an administrative fee of € 30,00 will apply; for cancellations received after 15th April 2007 a refund of 40% of registration fee will apply. After 15th May 2007 no refunds will be processed.

Refunds will be handled 4 to 6 weeks after the close of the conference.

Data Privacy Policy

AOPC informs you that all your personal data collected by the fulfillment of any electronic form, will be processed with the maximum confidentiality, according to the Privacy and Security Policy of this company, and with total compliance to the Personal Data Protection Law 15/1999, of 13 December.
AOPC has an automated file with personal data recorded in the General Record Department of the Spanish Data Protection Agency, as well as a documentary file. The purpose of its creation and maintenance is the treatment of the personal data in order to give compliance to the allowed objectives of the company, as providing our products and services, as well as the promotion of the organized congresses. By the fulfillment of any forms from this web site, you allow us to store your personal data for an automated and documentary treatment, as well as for its publication in the necessary documentation which will be edited during or after the preparation of the congress, even for future editions. By the other side, you allow us to, in case that it becomes necessary, communicate your data to the corresponding departments or collaborating organizations that participates in the organization of the event .
The requested data are the strictly necessary ones for the purposes before indicated, as well as to carry out basic tasks of administration and commercialization of our products and services.
